tcmdx64 loads shell extensions via OLE2, so it's very probable that one of these crashed. If this happens again, you can open the "x64" submenu while holding down the shift key to just show the "configure" menu item. Then you can exclude half of the shell extensions to find out which one causes the crash - if the there is still a crash, the bad extension is in the included part, otherwise in the excluded part of the extensions. This why you will find the bad one very quickly.
