1. Open Internal Associations dialog.
2. Double-click any of the entries to open its Edit dialog (pressing Edit button will not reproduce the bug).
3. Close the Edit dialog by pressing OK/Cancel button or Esc key.
4. The Internal Assiciations dialog is now inactive: the buttons are not highlighted when mouse moves over them (you need to use non-Classic theme to see this). If you click any of its buttons (e.g. OK) it is not clicked and nothing happens, but the dialog becomes enabled, so the next click works. It also becomes enabled after Alt+Tabbing from TC and back.

Maybe, video will help:
Video/EXE (player/codec included)
Video/AVI (needs MSU Screen Capture Lossless Codec)

Here I start a TC with fresh INI, create a new association (so there was what to double-click), double-click it, close the Edit dialog and show that buttons are not highlighted with blue and that the first click on OK does not close the Associations dialog.

I see, it only happens when you open the child dialog TWICE, and the second time must be by double click. I'm opening it in the OnDblClick event, which seems to cause that a message is not received by the dialog (probably the mouse up message). Opening the dialog via PostMessage seems to resolve the problem...

Actually, it can be only once, you just need to have an entry to double-click on (I had to create one at first because of the empty INI).
