This looks like a total commander bug:
If you change default DPI settings (Right click on desktop -> Display properties/Settings/Advanced/General/DPI settings) to some larger number than the default 96 DPI, total commander doesn't show the correct icons by the file names anymore - actually this happens only for some file types. In my case for pdf, avi, ps, ... files. Instead it shows some default icon.
In Windows explorer and other programs the icons are shown ok.
Any suggestions how to fix this?

This happens when *.ico is associated with a program, and the default icon for .ico is not %1 or "%1". Please try to change the default icon for .ico to %1 in Files - Associate - Edit type - Change icon.
Why is this done? Because ExtractIcon and ExtractIconEx return terribly distorted icons, so I have to use a different function.

- For instance, I've 21x21 icons for the lists, and *.ico displays 16x16 always, same for the system-dirs.
- I've reported that already a long while ago, any progress in sight ?
