Improvements to the "Cannot read" error dialog

The "Error:" prefix should instead be the error icon (stop sign) or perhaps the exclamation icon. Also, the "Skip" and "Cancel" buttons should not resize with the size of the dialog, which can change depending on the length of the name of the file that can't be read.
