MenuWithIcons: no icons for user em_*** commands

The behaviour described in the bug report is either by design, or would be far too complex/time-consuming to be changed

I didn't change it but I can post it here anyway if it's necessary :)
As far as I know, em_*** commands does not have numeric identifiers, more exactly they are not available for usual user. And moreover, how can it help?

Menu icons are not shown for user em_*** commands.
That is not implemented (yet) - only icons for internal commands that are stored in either wcmicons.dll or wciconex.dll and mapped in their respective .inc files are used in the menu.

But it would be nice if TC could read the icons for em_commands from the "button" section of each [em_command] in the usercmd.ini or the wcmd_xxx.ini file.

Currently menu icons are only supported for cm_ commands. Why? The problem is that it could take a LONG time to extract icons from all the EXEs etc referenced by these em_ commands or in the start menu. It's planned for a later TC version, but will require caching (like the button bar).
