Ctrl+Alt+ENTER can be used for that (Run As Administrator File Under Cursor).
Now is available only by using right click (Vista).

Also the button bar needs an entry in the right click menu for running as admin. Following your suggestion, Ctrl+Alt+Left Click could also do it.
This is needed in XP too.

Why not customize Ctrl+Alt+Enter to cm_OpenAsUser command?
