Problems w/icons in the Button Bar

if a Button Bar item refers to a program that is removed the item is missing an icon, leaving just an empty space. Only with a mouse above it you can detect that there is yet an item. I think it would be good to show some default icon in such cases.

That's indeed a good idea. Instead of a default icon it could indicate a broken link or similar.

Moreover, this icon should be "noticeable". For example: a solid black (or white) square, or some note like "No icon!", or even the "old" icon with a small red cross (in the same corner as a shortcut sign, being an arrow).
So, user could easily see that something is wrong with this icon.

Maybe it's really a good idea to distinguish between broken links to programs and broken links to icon.

IMHO the easiest way to see that something is missing - is that it is missing.

I do not use the "Flat icons" setting, so I is very easy for me to spot the empty icon buttons, because there is still a button.

I suggest that when "Flat icons" are enabled, TC draws the countour of where the icon should have been - that way it can be distinguished from separators.
