Improve help relating to Internal Associations

Go to Help and click on the Index tab. Then type "Associate". Then double-click on the first result "Associate". A window pops up where you can choose either "Associate" or "Dialog box: Associate". Suggestion:
- In order to be consistent, in the help index change "Internal Associations" into "Dialog box: Internal Associations"
