In TC's main window, if you press and release the Alt key then the menu has focus. You can visually see that.
In TC's dialog windows, if you press and release the Alt key then the "empty menu" gets focus. If you then press the arrow down key or space bar, you will see what I mean. When this empty menu has focus, it's not visible that the focus in not on the control with the focus rectangle. So, it's not visible why pressing Esc for example, doesn't work the first time.
(I don't know if this is normal or can't be helped.)

I just tried the same with Notepad.exe - same behaviour.
Press on Alt in the main menu sets focus on the first item in the menu.
Press on Alt followed by Arrow-down in a dialog box (fx. Help -> About Notepad) opens the little Move / Close window in the upper left corner.

Indeed it is used to reach the system menu. Seems to be standard Windows behaviour.

You mean main menu that only contains the system menu? The system menu is part of the main menu isn't it? In TC's main window or in Lister after pressing and releasing Alt, you can press arrow left and then arrow down to reach the system menu.

On Windows each and every window of native applications has a system menu (unless the application explicitely and intentionally removes it), but a main menu is completely optional. In other words, the system menu is not part of the main menu.
