Usability. Add new Custom column

If user want to add new custom columns, he press button "New".

If user wishes to add a new set of columns, he presses button "New".

How you think, he will press what button in dialogue "Custom columns"? Right! Add Column.

Why Total does not do it for the user? Not clearly.

User chooses field contents and wishes to add one more field. How you think, he will press what button? Right! Add Column.

In dialog always there should be a new empty field that at once it was possible to press [+]
