Confirmation dialogs bug
In Configuration/Misc I have Get confirmation before Overwriting files turned off, but the dialog is displayed anyway. TC 7.55a on Win7 x64.

Not confirmed, I have just tried it!
Are we talking about the same dialog? It's the dialog which can show thumbnails to the right of the source and target name.

It seems that the copy dialog options are pinned and the option 1. Ask user is selected.

I can see a bug here, when the copy dialog options are pinned, the option 1. Ask user is pre-selected regardless of overwriting files confirmation setting
Try the following:
1- Start TC with fresh ini
2- Turn off Overwriting files confirmation
3- Copy a file from left to right
4- In copy dialog click Options>> and notice that the option 2. Overwrite all is pre-selected
5- Pin the options and cancel the dialog
6- Repeat the copy again
7- Now you will notice that the option 1. Ask user is pre-selected not the option 2. Overwrite all
