organizing 'Association' (also adding app icon)

As you can see in pic, association make menu malformed. Mixing it with default windows open with sub-menu, or just making a new 'open with' is a good approach.

Also not having app's icons is sucks.

If you add Icons for Associations please make it optional as this will slow TC Portable down very much. TIA
