i'm using win98b-os here and expirienced similar occurances. since i'm using
a custom iconlibrary for totalcommander, i noticed that if the "read only" or
"system" attribute is active in any combination with the other attributes, you
get to see this plain yellow folder image just like in martijn's posted image.
i find this annoying and would welcome a solution to this behaviour.

maybe it's in christians hands to help us here?

With me, it has nothing to do with either the "read only" or "system" attribute. I turned all attributed off, on and off again, but no change.

If any of attributes R, H or S is set for a Directory then TC displays the icon the same as Explorer does. For experiments try to change the icon of folder (on XP - change icon, not image representing the folder) - afterwards the folder will get the attribute R and will contain the hidden file "desktop.ini". At least this happens on my computer.

2All:

And I think bahaviour of TC with icons is OK. If you like it - enjoy it.
If you do not want these icons but you only want TC's standard directory icons then just remove the attributes R H & S from this directory.

If someone comes across the case when it does not work then please, provide:
full name of the folder, attributes of folder (can be seen in Full mode (Ctrl+F2) ), contents of desktop.ini concerning icons (eg. IconFile=...)
