The builtin TC media player often (but not always) does not start playing a file, I need to push "play" manually. This happens usually with MIDI files, sometimes with MP3 or WAV.
Samsung Galaxy Nexus, all ROMs 4.3..6.0.1, all versions of TC since approx. 2.0.

I have seen this too, but couldn't find a pattern yet when this happens, sorry.
