Did you use a clean ini or did you disable the option. In case of the latter, you need to re-open the dialog.
Did you click the up/down buttons and it did nothing?

Not confirmed either, tested under Windows 10 and 7 with TC 11.50rc1 32bit and 64bit with clean ini in both normal and dark mode.
I also tried with TC 11.03 x64 under Windows 7 with clean ini - I still cannot reproduce the problem.o. And the issue isn't new for version 11.50.

Confirmed that there is no issue when I use a Windows 10 virtual machine. Thanks, it seems to be a bug of the Lazarus scrollbar control - the actual Windows SCROLLBAR control is enabled although the Lazarus TScrollbar control is disabled. This only happens when it should be initially disabled - clicking on "Set dots per inch (DPI) to" to disable the option also disables the scrollbar.

Note that the issue only happens when using Windows 11 (perhaps only 24H2?). Only in normal mode. And it happens when using 32-bit TC or 64-bit TC (the latter now fixed).

No one said anything about only 64-bit being effected. Not fixed in TC 11.5rc3 32-bit.
And I found another way to reproduce it instead of executing cm_FontConfig. Select menu option Configuration/Options, click category Color, click category Font.
